When the undercarriage was retracted after take-off from Winnipeg, the nose and left main undercarriage failed to retract. The pilot decided to
return to Winnipeg but was then unable to fully extend the left main undercarriage. After troubleshooting failed to rectify the problem, a landing
was made with the left main undercarriage only partly extended and it subsequently collapsed on touchdown. The aircraft came to a stop on the
runway resting on its underbelly cargo pod. The accident happened in darkness (1835L). The aircraft was operating a cargo flight to Sandy
Lake, Ontario.
A subsequent examination found that a bolt had sheared at an undercarriage linkage connection and the left main undercarriage actuator was
damaged.
